Bring valid government-issued photo ID and payment of $21 for the first certified copy and $4 for each additional copy.

Death records are registered at the county level and forwarded to the state DSHS, so both the county and state have the same records available from 1903 onward. For older death index research, FamilySearch offers a free Texas Death Index from 1903 to 2000. Ancestry has the same index with over 7 million death records and images for some years. Both are useful tools for identifying a record before you pay for a certified copy. The Texas State Library and Archives holds microfilm indexes for 1903 through 1973 as well.

Conroe is the county seat of Montgomery County, making the county clerk's office conveniently located within the city for in-person record requests.

In-person requests at the Montgomery County Clerk are typically processed quickly when the record is on file. You fill out the application with the required information listed above, show ID, and pay the fee. Mail-in requests to the county or to DSHS in Austin are accepted. State mail-in orders take 25 to 30 business days on average. Online orders through Texas.gov take 20 to 25 business days. Processing time does not include shipping.

To order through DSHS by mail, use the VS-142 application form. Download it at dshs.texas.gov and send it with payment to P.O. Box 12040, Austin, TX 78711-2040. For expedited processing, use overnight mail to 1100 West 49th Street, Austin, TX 78756, with the required expedited fee. Mail payment must be by check or money order made out to DSHS Vital Statistics. The county clerk may require a different local form, so call ahead if you plan to mail directly to the county.

Note: Under Texas Government Code ยง 552.115, death records from the past 25 years are restricted to qualified applicants only.

Who Can Request Conroe Death Records

Montgomery County follows state rules for who may request a death certificate. Only qualified applicants can get a certified copy of a death record from within the past 25 years. The registrant (if living), a parent, child, brother or sister, spouse, grandparent, legal guardian, managing conservator, or legal representative acting under contract for the registrant are all qualified. You must show valid ID and state your relationship on the application.

Death records older than 25 years are public. Anyone can request them. Verification letters are available for all deaths since 1903 and confirm the name, county, and death date. They are not certified copies and cannot serve as legal substitutes. If a record search comes up empty, a non-refundable search fee equal to the certificate fee applies. Use FamilySearch or Ancestry to confirm the record is on file before paying for a certified copy.

The full ID requirement list is at dshs.texas.gov. Primary Group A documents are preferred: Texas driver's license, state ID, U.S. passport, military ID, or license to carry a handgun.
